This opportunity supports investigator-initiated, single-site clinical trials addressing questions relevant to the National Heart, Lung, and Blood Institute (NHLBI) mission. Eligible trials include those focused on efficacy, comparative effectiveness, pragmatic, and/or implementation research, and may test therapeutic, behavioral, or prevention strategies. Innovative trial designs (e.g., adaptive, Bayesian) are encouraged. The award uses a two-phase, milestone-driven structure: an initial start-up phase (R61) and a full trial execution phase (R33), with progression dependent on achievement of predefined milestones.
The program is intended to accelerate the development and implementation of clinical trials that can inform clinical practice, health policy, or advance scientific knowledge in heart, lung, blood, and sleep disorders. Community engagement and strategies to reduce health disparities are strongly encouraged.

Letter of Intent: 30 days prior to the application due date.
Earliest project start dates: Typically 6–9 months after submission, following review and council meetings.

Individuals: Any qualified investigator(s) with the skills, knowledge, and resources to carry out the project. Early Stage Investigators with clinical trial experience are encouraged to apply (with appropriate mentorship).
