Description
This award honors an individual’s outstanding commitment and contribution to the delivery of audiology and/or speech-language pathology services in a school setting. Established in memory of Rolland J. Van Hattum, the award celebrates professionals who demonstrate innovation, leadership, and a significant impact on school-based services. The recipient receives a $2,000 award and a commemorative plaque, while their school system receives $500 to further the awarded work.

Eligibility
- Who can be nominated:
- Audiologists or speech-language pathologists employed by a school system (serving pre-K through high school).
- Individual or agency contractors are not eligible.
- Contribution requirements:
- The nominee’s contribution must be made within a school system and have a direct, observable, and demonstrable impact.
- The commitment must be evidenced by a specific activity or achievement, or a series of related activities with a common goal.
- The contribution must be clearly outstanding (e.g., innovative, creative use of resources, collaborative).
Application Process
- Nomination and Summary Form:
- Complete the online form with nominee and nominator contact information and a brief summary statement (three sentences or fewer).
- Nomination Document (PDF, max 8 pages):
- Nomination Narrative: Biographical introduction and summary of educational/career activities.
- Specific Contribution: Detailed description of the nominee’s contribution, its impact, and the population served (use provided narrative headers).
- Case for Support: Letters or statements from professionals and, if appropriate, service recipients or their families.
- Formatting:
- Single PDF, 12-point uncondensed font, 1-inch margins, single-spaced, maximum 8 pages. Excess pages will be removed.
