This program supports organizations in serving as a bridge between the Illinois Juvenile Court System, probation, schools, and community organizations to identify and refer youth with, or at risk of, mental health concerns. The MHJJ Liaison, employed by the grantee, coordinates with youth, families, managed care organizations, and referral sources to link youth and their families to appropriate community-based mental health and social services. The program emphasizes equity, diversity, and inclusion, and requires ongoing case monitoring, engagement plan development, and training for referral sources.
