Description
This opportunity provides funding to HRSA-supported health centers to increase access to nutrition services as part of comprehensive primary care. The aim is to prevent, manage, and treat chronic diseases and conditions through nutritional and food-based interventions. Projects must propose to increase the number of nutrition services patients or visits, and may include nutritional assessments, counseling, medical nutrition therapy, group programs, and related patient support services. Funding may also support minor facility improvements or equipment needed to expand nutrition offerings, as well as partnerships with community organizations to improve patient access to healthy food.
