This funding opportunity supports the "240 Successful Transitions" pilot program, which aims to assist individuals being discharged from Illinois State-Operated Psychiatric Hospitals (SOPHs) in reintegrating into their communities. The program provides up to 90 days of pre-discharge and up to six months of post-discharge non-clinical support, delivered by Transition Support Specialists with lived experience in mental health or co-occurring substance use recovery. Services include mentoring, education, advocacy coaching, and tangible resources such as transportation, clothing, and cell phones. The overarching goal is to reduce re-hospitalization, support wellness and recovery, and facilitate connections to treatment, employment, housing, and healthcare.
