Description
The Office of Minority Health, under the Office of the Assistant Secretary for Health (OASH), announces funding for Fiscal Year 2024 to support community-level innovations aimed at improving health outcomes by addressing social determinants of health (SDOH). The goal is to demonstrate that reducing barriers related to SDOH can increase the use of preventive health services and advance progress toward Leading Health Indicator (LHI) targets, which are priority objectives from Healthy People 2030 (HP2030).
SDOH are defined as the conditions in the environments where people are born, live, learn, work, play, worship, and age that influence a wide range of health, functioning, and quality-of-life outcomes and risks.
This grant opportunity anticipates awarding up to 14 grants, each ranging from $475,000 to $600,000 annually, for a period of up to 48 months. Recipients must report progress and milestones annually through noncompeting continuation applications. Medical service costs are not allowable expenses under this funding.
Early in the fourth budget period, a competing continuation opportunity may be offered for an additional 12-month budget period to support successful projects transitioning to sustainability. However, funding for this additional period is not guaranteed and may be at a reduced level.
The OASH Grants and Acquisitions Management Division (GAM) will provide administrative support for this competition.
