DescriptionThe purpose of this program is to provide diabetes treatment and/or prevention activities and/or services (also referred to as “activities/services”) for AI/AN communities. Recipients will implement one Special Diabetes Program for Indians (SDPI) Diabetes Best Practice (also referred to as “Best Practice”) and report data on the Best Practice’s Required Key Measure (RKM).
