DescriptionThe Centers for Medicare & Medicaid Services (CMS), through its Center for Medicare and Medicaid Innovation, is soliciting applications for the Transforming Maternal Health (TMaH) Model. State Medicaid Agencies (SMAs) for the 50 states, District of Columbia, Puerto Rico, the U.S. Virgin Islands, Guam, American Samoa, and the Commonwealth of the Northern Mariana Islands are eligible to apply. Applicants must propose either to implement the Model statewide or to implement the Model in a sub-state region specified by ZIP codes.
