Key Information: No cost sharing/matching required; only pre-approved programs from ACL lists are eligible.
This opportunity funds up to five cooperative agreements to develop, deliver, and sustain statewide, evidence-based chronic disease self-management education (CDSME) and support programs targeting older adults (60+) and adults with disabilities experiencing behavioral health conditions or stressors. The initiative aims to increase participation in CDSME programs, build partnerships between aging/disability and behavioral health networks, and disseminate learnings and resources to support replication and sustainability.
Projects must implement at least one CDSME program and one self-management support program from ACL's pre-approved lists, with a focus on person-centered, trauma-informed, and culturally appropriate delivery. The target population includes older adults and individuals with disabilities who have behavioral health conditions (e.g., depression, anxiety, PTSD, substance use) or stressors negatively impacting behavioral health.
