This program aims to prevent unnecessary institutionalization of adults at risk of admission to Specialized Mental Health Rehabilitation Facilities (SMHRFs) in Illinois by providing rapid, community-based behavioral health, housing, and support services. Through individualized diversion plans, the project seeks to maintain eligible individuals in the most integrated community settings. Funded services include comprehensive clinical and housing assessments, transitional housing, behavioral health and substance use disorder treatments, peer and employment supports, and assistance with securing permanent housing and benefits. Grantees must ensure outreach, education for referring entities, and ongoing participant support, while adhering to strict performance, reporting, and quality assurance standards.
