This program aims to divert adults (18+) who would otherwise be referred or admitted to a Specialized Mental Health Rehabilitation Facility (SMHRF) in Illinois into integrated, community-based settings. Through rapid engagement following PASRR assessment and referral, providers assess clinical, housing, and support needs within 24 hours and offer individualized diversion plans. Services include behavioral health and substance use disorder treatment, transitional and permanent housing support, and ongoing care management. The program focuses on maintaining individuals in the least restrictive environments and targets populations with serious mental illness, co-occurring disorders, forensic involvement, homelessness, and those aging out of state care. It operates in select Illinois counties and is aligned with the Williams Consent Decree to expand diversion statewide.
