Description
This opportunity, offered by the Administration for Community Living (ACL), funds cooperative agreements to develop and expand dementia-capable Community Health Worker (CHW) programs within the National Aging Network. The goal is to enhance workforce capacity by integrating CHWs—trusted community members trained to address barriers to care—into home and community-based services for people living with dementia and their caregivers, especially in rural and underserved communities. Projects must implement evidence-based or evidence-informed dementia interventions, prioritize direct services, and include robust training and third-party evaluation components.
