This opportunity provides cooperative agreements to support the development and expansion of dementia-capable home and community-based service (HCBS) systems in states and communities. The program aims to help individuals living with Alzheimer’s disease and related dementias (ADRD), along with their caregivers, remain independent and safe in their communities through person-centered services and supports.
There are two application tracks:
Applicants may only apply for one option per cycle and may not hold more than one ADPI grant at a time. Priority consideration is given to states and organizations that have not received similar funding since before 2014.
