This program funds cooperative agreements to help states and communities develop or expand dementia-capable home and community-based service (HCBS) systems. The goal is to improve the quality of life for individuals living with Alzheimer’s disease and related dementias (ADRD) and their caregivers by strengthening support systems, closing service gaps, and ensuring access to person-centered, high-quality services. The initiative offers two application options: Grants to States (Option A) and Grants to Communities (Option B). Applicants may apply for only one option and may not hold more than one ADPI grant at a time.
