Description
This opportunity, administered by Iowa Health and Human Services (Iowa HHS), aims to fund organizations to initiate or expand Mobile Integrated Health (MIH) services in rural areas across Iowa. The goal is to enhance access to care, chronic disease management, post-discharge follow-up, and community-based clinical interventions for rural Iowans.
