The Rhode Island Department of Health’s Diabetes and Cardiovascular Health Program is funding short-term projects to implement the Healthy Heart Ambassador Blood Pressure Self-Monitoring program in Rhode Island communities. Organizations may deliver the program virtually, in person, or through a combination of both.
The four-month program trains participants to monitor their blood pressure and includes twice-monthly facilitator check-ins and four monthly nutrition seminars. It is part of the department’s work to reduce death and disability due to diabetes and heart disease through evidence-based health promotion.
