Description
The Virginia Department of Health (VDH) Office of Emergency Medical Services administers this opportunity under Virginia’s Rural Health Transformation Program. It supports community-driven, evidence-informed paramedicine and mobile integrated healthcare projects that serve eligible rural Virginia communities. The goals are to expand access to patient-centered care, improve coordination and health outcomes, and preserve EMS resources for emergency response.
Projects may establish, expand, or adapt services such as treat-in-place care, chronic disease management, post-discharge follow-up, preventive care, behavioral health support, and patient navigation. VDH seeks sustainable models that integrate EMS with healthcare and community partners, respond to locally identified needs, and could be replicated or expanded elsewhere in rural Virginia.
