Description
This opportunity aims to improve healthcare delivery in the Delta region by supporting the planning and development of integrated networks among rural hospitals, primary care clinics, behavioral health providers, and other essential healthcare organizations. The program funds both new and existing networks to enhance collaboration, efficiency, access, and care quality for rural communities in the Delta Regional Authority (DRA) area. Applicants must select one of two tracks: (1) creating a new network or (2) strengthening/expanding an existing network.
