This program funds the planning, development, and implementation of integrated health care networks in rural areas of the eight Mississippi Delta Region states (Alabama, Arkansas, Illinois, Kentucky, Louisiana, Mississippi, Missouri, and Tennessee). The goal is to achieve efficiencies, expand access, coordinate, and improve the quality of basic health care services, and strengthen the rural health care system as a whole. Projects must focus on no more than two priority health areas, such as diabetes, cardiovascular disease, obesity, stroke, chronic lower respiratory disease, cancer, or unintentional injury/substance use, and employ evidence-based or promising practice models.
Funded networks are expected to address local health challenges, expand access, improve health outcomes, and implement sustainable, collaborative programs. Awardees will participate in technical assistance activities and must collect and report longitudinal data to demonstrate impact.
