The initiative builds upon the ACL Innovation Lab and seeks to scale interventions through community care hubs and their clinical partners, focusing on the aging services network. The successful applicant will collaborate closely with ACL to design and implement demonstrations that support care transitions, risk screening for falls, and chronic disease management. Emphasis is placed on rapid cycle evaluation, the use of artificial intelligence, data analytics, assistive technology, virtual delivery, and tools to support consumer behavior and data infrastructure. The grantee must also be able to administer sub-awards to community care hubs capable of implementing and scaling these interventions.
