    Description

    The Continuum of Care (CoC) Program, administered by the U.S. Department of Housing and Urban Development (HUD), funds community-based efforts to end homelessness. The program aims to quickly rehouse individuals and families experiencing homelessness, minimize the trauma and dislocation caused by homelessness, and promote access to mainstream resources and self-sufficiency. The Youth Homelessness Demonstration Program (YHDP), included in this opportunity, supports the development and implementation of coordinated community approaches to prevent and end youth homelessness, targeting youth up to age 24, including unaccompanied, pregnant, or parenting youth.

    Funding is awarded competitively to support a broad range of housing and supportive service activities, including permanent supportive housing, rapid re-housing, transitional housing, and coordinated entry systems. Collaborative Applicants are responsible for submitting consolidated applications on behalf of their communities.
