--- ## Description This opportunity provides funding to state governments for planning activities related to establishing Medicaid health home programs for individuals with chronic conditions, as authorized under Section 1945 of the Social Security Act and the Affordable Care Act. The grant supports states in developing State Plan Amendments (SPAs) to coordinate care for Medicaid beneficiaries with multiple chronic conditions, at risk of developing additional conditions, or with serious and persistent mental health issues.
