The Massachusetts Community Health and Healthy Aging Funds provide significant grant support to organizations and communities in Massachusetts that are addressing the root causes of health inequities. The program invests in upstream policy, systems, and environmental change (PSE) approaches, community health improvement planning (CHIP) processes, and healthy aging initiatives. Its vision is to transform systems across sectors, prioritizing people of color and older adults, so that all Massachusetts residents have equitable opportunities for the highest quality of life.
The Funds are both a grantmaking and capacity-building resource, supporting innovative and sustainable strategies that tackle social determinants of health and disrupt structural barriers. Applicants are encouraged to propose creative, community-driven projects that address inequities through policy, systems, or environmental changes.
