This opportunity will fund one organization to serve as the National Chronic Disease Self-Management Education (CDSME) Resource Center under a cooperative agreement with the Administration for Community Living (ACL). The Resource Center will act as a central connector for the aging network, offering individualized technical assistance, high-quality education, and extensive resources to increase participation in evidence-based CDSME and Self-Management Support Programs. The Center’s goals include building partnerships, supporting data collection and reporting, highlighting achievements, and engaging stakeholders to ensure long-term sustainability and cost-effective delivery of services aligned with the Older Americans Act, Title III-D.
