This opportunity will fund a single organization to serve as the National Chronic Disease Self-Management Education (CDSME) Resource Center under a cooperative agreement with the Administration for Community Living (ACL). The Center will act as a national hub, supporting the aging and disability network by delivering individualized technical assistance, expert-led training, and accessible resources to increase the reach and impact of evidence-based CDSME and self-management support programs. The Center will also build and maintain a secure, Section 508-compliant cloud database to support data collection, reporting, and continuous quality improvement, with the ultimate goal of improving health outcomes and independence for older adults and adults with disabilities.
