Description
The New Jersey Department of Community Affairs’ Office of Homelessness Prevention, in partnership with the Division of Medical Assistance and Health Services, is funding a limited number of hospital and Housing Supports Program provider partnerships. The pilot aims to identify NJ FamilyCare members experiencing housing instability in healthcare settings, engage them before discharge, and connect them to ongoing housing case management in the community. Hospitals and acute care settings are the primary focus, though other healthcare settings may be considered.
Partnerships may develop technology and secure information-sharing systems, referral and engagement workflows, operational practices, workforce capacity, and outreach. The pilot will test approaches to hospital–housing integration and generate lessons that could inform a replicable model across New Jersey. A separate solicitation funds the pilot’s technical assistance provider; this grant is for participating healthcare and Housing Supports partners.
