The New Jersey Department of Community Affairs’ Office of Homelessness Prevention administers this grant in partnership with the Division of Medical Assistance and Health Services. It will select an organization to help hospitals and NJ FamilyCare Housing Supports Program providers launch and operate a separate hospital–housing integration pilot. The provider will help partners establish agreements and referral workflows, train staff, resolve operational challenges, and share learning across pilot sites.

The provider will also conduct a mixed-methods assessment of how different integration models work in practice. That assessment will draw on hospital and housing provider staff, participating NJ FamilyCare members, oversight staff, and feasible descriptive analysis of pilot data. Interim and final findings will identify effective practices and barriers to inform decisions about refining or expanding coordination between healthcare and housing systems.
