Description
The New Jersey Department of Health’s Outreach and Education program supports work with healthcare providers and communities to increase awareness of chronic disease in maternal health. Funded activities identify risk factors and implement evidence-based preventive strategies to decrease the incidence of chronic disease.
The program also supports provider education on the Perinatal Risk Assessment and onboarding and training in the PRAConnect data system. Other activities include helping people quickly access information about New Jersey healthcare services and resources; providing education, training, and support for the Healthy Women, Healthy Families resource and referral system; and collecting, managing, and analyzing data.
