--- ## Description This program provides funding for community-based organizations and federally recognized tribal governments to deliver mentoring and transitional services to adults returning to their communities following incarceration in prison or jail. The initiative targets individuals assessed as moderate to high risk for recidivism and aims to support their successful reintegration and reduce the likelihood of reoffending.
