Description
This funding opportunity from the Agency for Healthcare Research and Quality (AHRQ) aims to establish State-based Healthcare Extension Cooperatives (Cooperatives) that accelerate the dissemination and implementation of patient-centered outcomes research (PCOR) evidence into healthcare delivery, specifically targeting behavioral healthcare improvements for medically underserved populations. The initiative focuses on reducing healthcare disparities among Medicaid recipients, uninsured individuals, and other underserved groups.
The Cooperatives will engage key stakeholders such as Medicaid agencies, managed care organizations, safety net healthcare delivery organizations, clinicians, patients, and community partners to identify barriers and facilitators to implementing evidence-based healthcare improvements. They will build capacity within healthcare policy, payment, community, care delivery, and research organizations to support ongoing learning and practice transformation.
Each Cooperative must include three cores:

Engagement, Training, Education, and Assistance Core: Establishes relationships with safety net organizations, provides training and tailored implementation support through healthcare extension agents, and facilitates statewide learning communities.

Monitoring, Feedback, and Evaluation Core: Conducts rigorous, independent evaluation of the Cooperative’s activities and initiative impact, including data collection and analysis of state-level healthcare data, and collaborates with the National Evaluation Center (NEC).

Administrative Core: Manages the Cooperative’s operations, governance, and staffing; establishes and facilitates a Multistakeholder Council to provide expert guidance; coordinates with other state and federal initiatives; and collaborates with the National Coordinating Center (NCC) and NEC.
The initiative must focus on behavioral health, addressing issues such as mental health, substance use, and related health behaviors. Applicants are expected to select a behavioral health focus based on state data, PCOR evidence, and stakeholder input, and to design an evidence-based, patient-centered improvement initiative that is sustainable and addresses health equity.
The Cooperatives will work closely with AHRQ and recipients of two related NOFOs: the National Evaluation Center (NEC) and the National Coordinating Center (NCC) for AHRQ’s Healthcare Extension Service.
