Description
This program funds telehealth networks to integrate nutrition services into primary and specialty care, targeting rural and medically underserved communities. Projects should focus on evidence-based interventions to prevent and manage chronic diseases and must demonstrate innovative approaches to expanding nutrition access, particularly for populations facing barriers due to geography, provider shortages, or other social determinants of health.
