Description
This opportunity provides cooperative agreements to federally recognized tribes, tribal organizations, and tribal consortiums to build or expand dementia-capable home and community-based services (HCBS) for Tribal Elders and their caregivers. The program aims to increase the capacity of tribal HCBS systems to deliver strengths-based, person-centered, and culturally competent services and supports for people living with dementia and their caregivers, helping them remain independent and safe in their communities. There are two application tracks: one for tribes/organizations new to the Alzheimer’s Disease Programs Initiative (ADPI), and one for those expanding previously funded ADPI projects. All projects must include at least one dementia-specific evidence-based or evidence-informed intervention and a robust third-party evaluation.
