This Illinois Department of Human Services (IDHS) grant aims to reduce barriers for individuals with disabilities transitioning from nursing facilities to community-based living under the Colbert & Williams Consent Decrees. The program funds comprehensive evaluations, mobile assistive technology, adaptive equipment, and home/worksite modifications to maximize independence and safety. Services are delivered in phases—addressing mobility, accessibility modifications, and instrumental activities of daily living (IADLs)—and include ongoing follow-up, training, and technical assistance. The grantee is also responsible for reporting outcomes and supporting the creation of accessible housing stock.
