Description
This program funds organizations to oversee and manage the transition of Class Members from nursing facilities or specialized mental health rehabilitation facilities to community-based settings in Illinois. Grantees are responsible for the entire transition process, from referral through an 18-month post-transition period, ensuring seamless, safe, and successful moves with limited handoffs. Services include care management, transition coordination, SSI/SSDI benefits assistance, integrated healthcare (nursing and occupational therapy), and administration of transition assistance and flexible funds.
