Description
The John A. Hartford Foundation’s New Partners Program brings new organizations, leaders, and ideas into its work to improve care for older adults and support family caregivers. It supports national or multi-site initiatives aligned with Age-Friendly Health Systems, Family Caregiving, or Serious Illness and End-of-Life Care.
The program seeks evidence-informed models of care or policy implementation activities that introduce innovative approaches or build on existing work in new ways. Projects should be scalable, sustainable, and centered on what matters to older adults and caregivers. Topics and settings are not prescribed. Areas of interest include rural access, technology, value-based payment, care coordination, social isolation, and dementia caregiving. The emphasis is on improving care at scale rather than principally delivering routine direct services.
