Description
The Administration for Community Living (ACL) plans to fund cooperative agreements that develop and expand dementia-capable community health worker (CHW) programs within the National Aging Network. Projects will integrate CHWs into community-based aging services.
The initiative aims to improve dementia screening and referral, deliver dementia-specific evidence-based or evidence-informed interventions, and reduce barriers to care. Its goals also include improving access to supportive services and quality of life for people living with dementia and their caregivers.
