    Description

This CDC National Center for Chronic Disease Prevention and Health Promotion program supports organizations strengthening national education and awareness for chronic diseases not supported through disease-specific CDC funding. Through cooperative agreements, it supports enhanced surveillance, work addressing priority evidence gaps, evidence-based educational resources, strategic partnerships, and adoption of evidence-based prevention, screening, referral, treatment, disease management, and self-management practices.

Recipients will review and disseminate surveillance data and evidence-based strategies, develop or refine survey tools, and create educational resources for public health professionals, health care professionals, and the public. Partnerships should expand the reach and use of evidence-based information. The program aims to improve access to education and awareness resources, strengthen public health information dissemination, increase use of evidence-based practices, and improve outcomes for people living with chronic diseases.
