Description
This program provides funding to support the expansion and improvement of primary health care services in Pennsylvania. It aims to reduce unnecessary use of hospital emergency departments and encourages collaborative relationships among community-based health care clinics, hospitals, and other health care providers. Funding is available for new clinic development, expansion or improvement of services at existing clinics, maternal and newborn care, alternative health delivery systems, and collaborative initiatives.
