Description

The Community Grants for Health & Wellness program, offered by the Hampton Roads Community Foundation, funds nonprofit initiatives that improve health outcomes for residents of South Hampton Roads, Virginia. The grant prioritizes projects that enhance access to primary, dental, and behavioral health care (including substance abuse treatment), and promote healthy behaviors to prevent or manage chronic diseases such as diabetes, heart disease, and lung disease. Special consideration is given to programs addressing health disparities and advancing health literacy among underserved populations. Funding can be used for seed or program support, as well as minor equipment needs that are part of a larger health initiative.
