Description
Community Health Grants are administered by the NCDHHS Office of Rural Health (ORH), with support from the North Carolina General Assembly. The program strengthens the health care safety net through collaboration and integrated services, emphasizing access for uninsured and medically indigent patients. It supports expanding access at existing locations, establishing services where none exist, sustaining services, and increasing primary care capacity.
Supported work includes health promotion, maintenance and counseling, disease prevention, patient education, and diagnosis and treatment of acute and chronic illnesses. Eligible medical homes may support integrated dental, pharmacy, maternal, and behavioral health services. Other supported activities include women’s and child health in primary care settings, community-based whole person-centered care, telehealth, Community Health Workers, and care coordination and navigation, including Medicaid Expansion activities.
