Description
The Community Health Matching Grant Program is administered by the Blue Cross Blue Shield of Michigan Foundation. It partners with health professionals and community leaders at nonprofit organizations to develop population-led, community-inspired programs or implement evidence-based programs addressing community-identified needs. Projects should evaluate measurable programmatic and health outcomes, including how their work addresses non-medical drivers of health.
The Foundation prioritizes health literacy and numeracy, senior health and caregiver support, nutrition and food access, healthcare access and quality, behavioral health and substance use disorders, maternal and child health, non-medical drivers of health, and health promotion and chronic disease prevention. Proposals also address implications for healthcare cost, quality and access, sustainability, and potential replication.
