Description
Texas Health and Human Services, on behalf of the Department of Family and Protective Services (DFPS), seeks Single Source Continuum Contractors (SSCCs) to expand Community-Based Care into catchment areas 7A and 7B. This performance-based model engages local communities in improving safety, permanency, and well-being for children and youth in DFPS legal conservatorship.
Contractors must provide a full continuum of substitute care, including foster and kinship care, purchased services, case management, and reunification support. Responsibilities extend to children from the designated community and those placed there through the Interstate Compact on the Placement of Children or interregional agreements. The model prioritizes individualized services, placement stability, family connections, and community engagement. Contractors must also participate in Texas Child-Centered Care (T3C), including its provider credentialing, assessment, placement, and quality-improvement requirements.
