Description
The Texas Health and Human Services Commission is requesting applications on behalf of the Department of Family and Protective Services (DFPS) to expand Community-Based Care into catchment areas 7A and 7B. Through a performance-based Single Source Continuum Contractor (SSCC), DFPS aims to engage local communities in improving the safety, permanency and well-being of children in its legal conservatorship.
The contractor will ensure a full continuum of foster and kinship care, purchased services, case management and reunification services. Its responsibilities cover children and youth from the designated area, as well as those placed there through the Interstate Compact on the Placement of Children or interregional agreements. The contractor must also participate in the Texas Child-Centered Care (T3C) model and its credentialing transition.
