    Description

    The New Jersey Department of Health’s Diabetes Capacity Building program funds evidence-based strategies to prevent and manage prediabetes and diabetes among adults in high-burden populations and communities across New Jersey.     The program is a continuation of the Preventive Health and Health Services Block Grant (PHHSBG) Diabetes Capacity Building Program. Its funding is directed to applicants previously supported through that program.
