Description
The Dr. Dorothy Brooten Transitional Care Research Grant is designed to advance evidence-based transitional care within clinical settings. The program supports nursing-led research projects that focus on improving the transition of patients between different sites or levels of care, aiming to enhance patient outcomes through innovative, evidence-based interventions. Proposals are evaluated based on the project's background and significance, problem statement, evidence synthesis, goals and timelines, potential for measurable improvement, and clear implementation and evaluation plans. The grant is awarded annually to one recipient.
