Description
The SCAN Foundation uses Driving Systems Change to improve the policies, programs, and systems that help older adults age well at home and in their communities. Its grantmaking starts with what older adults say they need, particularly people whose perspectives have historically been left out of decisions. Partnerships with community organizations, advocates, and leaders with lived experience help those affected shape services and solutions.
The foundation also funds inclusive data and research that can inform policymakers and program leaders, cross-sector coalitions working toward policy change, and storytelling and knowledge-sharing that draw attention to effective approaches. Its priorities include long-term care, Medicare-Medicaid integration, Medicare health and social supports, and Multisector Plans for Aging.
