    Description

    This fund supports nonprofit-led programs that foster healthy individuals, families, and communities by reducing the incidence and prevalence of chronic kidney disease and related risk factors such as obesity, diabetes, and hypertension in the Greater Capital Region of New York. The fund prioritizes initiatives that promote healthy habits and lifestyles (nutrition, physical activity, tobacco cessation, sleep, stress reduction) and strengthen health equity, especially for vulnerable populations including Black and Latino/x communities, urban and rural residents. Programs that address sustainable food systems, access to quality healthcare, and essential needs (housing, transportation, food) are encouraged. Special consideration is given to organizations providing services for those with chronic kidney disease, particularly those focusing on home dialysis.
