Description
The FY27 Medical Homes Grants (Medical Home Model Grants) are designed to foster the development and implementation of the medical home model for individuals with Autism Spectrum Disorders (ASD) in New Jersey. This program, funded by the New Jersey Governor's Council for Medical Research and Treatment of Autism, aims to support innovative, patient-centered, and family-focused approaches that enhance care coordination, comprehensive health services, and improved outcomes for people with ASD and their families.
