Description
The Greater Rochester Community Health Foundation Grant provides general operating support to nonprofit organizations working to address access to care challenges for residents of Strafford County, New Hampshire and surrounding communities. The grant aims to improve health and well-being, and reduce illness burden, by supporting services such as substance use disorder care, food security, housing, senior services, transportation, culturally appropriate care, service awareness, care coordination, and patient motivation. Priority is given to collaborative projects and initiatives targeting maternal health.
