Description
The Health Disparities and Community Grant Program (HDCGP) is administered by the Colorado Department of Public Health and Environment (CDPHE) and funded through Colorado state tobacco tax revenue (Amendment 35) and appropriations from the state legislature (SB21-181). The program aims to address and reduce health disparities in Colorado by supporting community-driven initiatives that promote health equity, with a focus on underrepresented and vulnerable populations.
The HDCGP operates two main funding portfolios:
- Amendment 35 (A35): Focuses on preventing, detecting, and treating cancer, cardiovascular, and chronic pulmonary diseases among underrepresented Coloradans.
- SB21-181: Supports projects that address social determinants of health (SDoH) to reduce the risk of future disease and health disparities.
Projects are expected to align with the Healthy People 2030 Social Determinants of Health framework and demonstrate authentic community engagement, systems or policy change, and measurable impact on health outcomes.
