Description
The Health Disparities and Community Grant Program (HDCGP) supports initiatives that reduce health disparities and advance health equity across Colorado. Funded by Colorado state tobacco tax revenue (Amendment 35) and legislative appropriations (SB21-181), the program targets community-based, policy, and systems-level interventions that address social determinants of health (SDoH) and improve outcomes for underrepresented populations. Projects must focus on prevention, early detection, or treatment of cancer, cardiovascular, and pulmonary diseases, or address broader SDoH to reduce risk and inequity.
Oversight is provided by the Health Equity Commission, with funding decisions subject to approval by the Colorado State Board of Health and legislative appropriations.
