Description
The Healthy Aging Program funds patient-based and social service activities that directly help older adults in New York City. The Fan Fox & Leslie R. Samuels Foundation prioritizes access to care and support for people with acute or chronic illness or palliative care needs, services for immigrant and economically disadvantaged older adults, and programs that help people remain at home or improve life in long-term care facilities.
The Foundation also supports improvements in how professionals and paraprofessionals care for older adults. Programs must meet a demonstrable need and deliver services or change provider or consumer behavior. Priority goes to work that can continue and grow after the Foundation’s funding ends.
