Description
This grant opportunity, managed by the Delaware Community Foundation through the Highmark BluePrints for the Community Fund, is designed to support large-scale, health-related projects and services that increase access to care and/or reduce health disparities across Delaware. Eligible projects must address at least one of five focus areas: healthcare access, economic stability, social and community context, neighborhood and built environment, or the health workforce. The program prioritizes initiatives that serve uninsured, underinsured, and historically underserved populations, with a focus on measurable improvements in health outcomes and equity.
