Description
Healthy Communities Delaware (HCD), administered through the Delaware Community Foundation, supports projects that advance community-level change across Delaware. Its mission is to work with communities, investors, and systems to create thriving places for thriving people. Funding focuses on improving the conditions that all people and communities need to thrive, rather than a single service area.
The program uses the Vital Conditions framework to guide its work. These priorities encompass humane housing, lifelong learning, a thriving natural environment, meaningful work and wealth, basic needs for health and safety, reliable transportation, and a sense of belonging and civic muscle. Projects should align with HCD’s shared mission and strengthen these conditions for Delaware’s people and places.
