Description
The Ohio Commission on Minority Health funds Infant Mortality Hub Grants to replicate and expand the Pathways Community HUB Model in communities across Ohio. The evidence-based model coordinates community care and has demonstrated effectiveness within racial and ethnic populations. Commission funding supports both replication grantees and new expansion grantees.
The model provides a framework for community health workers to identify and address health and social risks, coordinate local resources, and track outcomes. The Pathways Community HUB Institute oversees certification of funded HUBs. All Commission-funded programs and activities must be free and open to the public.
